How do I submit an FOI Request?
Your application should be in writing and addressed to:
FOI Officer,Freedom of Information Unit, Office of Public Works, Jonathan Swift Street, Trim, Co. Meath
and must be accompanied by the appropriate fee. Details of fees are contained on the FOI application form.
Payment should be made by way of bank draft, postal money order or personal cheque made payable to "The Office of Public Works". If claiming a reduced application fee, the request must also be accompanied by the Medical Card registration number, issuing Health Board and your consent to the verification of these details with the Health Board.
Your letter should:
· refer to the FOI Act
· give as much information as possible about the records being sought
· specify if you would like to obtain copies of the record or would prefer access in another form
Alternatively, you may use the application form available below and submit this along with the appropriate fee to the FOI Officer at the address given above.
